This link ( should take you directly to a pdf file that will give you step by step instructions and all of the forms necessary to change your name in New Jersey. The only info I would add to this form is:

1. Deliver all of your forms in person. (This will mean two additional trips to the courthouse), or at the very least send all documents 'return receipt'

2. Look as professional as possible at your hearing.

I had no problem at all changing my name.
